Introduction: Thank you for choosing R Bansal Medical PLLC ("we," "us," "our," "the Practice", or "Halt Clinic") for your healthcare needs. As a telemedicine-only practice, we provide all medical services remotely through secure digital platforms. By using our services, you agree to the following terms.
1. Nature of Telemedicine Services
Telemedicine is the practice of medicine using electronic communications, information technology, or other means between a healthcare provider and patient who are not in the same physical location.
Our Services Include:
- Initial consultations and evaluations
- Follow-up appointments
- Medication management and prescription services
- Health education and counseling
- Review of diagnostic tests and imaging
Important Limitations:
- No Physical Examinations: We cannot perform hands-on physical examinations, laboratory tests, or procedures.
- Emergency Care: We do not provide emergency medical services. In case of emergency, call 911 immediately.
2. Patient Responsibilities
By using our services, you acknowledge and agree to the following responsibilities:
- Technology: You are responsible for ensuring adequate internet connectivity and compatible devices (camera/microphone).
- Location: You must be in a private, well-lit location for the consultation.
- Accuracy: You must provide an accurate and complete medical history.
- Identity: You must verify your identity when requested.
3. Financial Policy & Cancellations
Halt Clinic operates on a self-pay basis and does not accept or bill insurance. Payment is due at the time of booking unless otherwise arranged in writing.
Cancellation Policy
- Free Cancellation: Cancellations made within 24 hours of booking are eligible for a full refund.
- Late Cancellation Fee: Cancellations made after 24 hours of booking, or rescheduling requests with less than 24 hours notice, will incur a $68 cancellation fee.
- No-Shows: Missed appointments without notice are non-refundable and subject to the full appointment fee.
4. Prescribing Policies
Prescriptions will be sent electronically to your preferred pharmacy. Please note:
- We do not prescribe controlled substances for initial patients without established care relationships.
- Controlled substance prescribing is limited in compliance with New York State and federal regulations.
- We reserve the right to decline prescription requests based on clinical judgment.
5. Emergency Procedures
In case of a medical emergency, CALL 911 IMMEDIATELY or go to the nearest emergency room. For urgent but non-emergency situations during business hours, please contact our practice.
6. New York State Requirements
All providers are licensed to practice medicine in New York State. This telemedicine service establishes a valid provider-patient relationship under New York State law.
